Your first visit to our office establishes a vital foundation for our relationship with you and gives you time to get to know Dr. Sergio and his wonderful staff.
This appointment will consist of a thorough examination. Whether a limited area or the complete mouth, we will obtain and review your medical and dental history, do an oral cancer screening and check your blood pressure. Current x-rays will be required for most procedures and can either be borrowed from your general dentist or take in our office, if necessary.
Treatment options will be discussed and once a treatment plan has been established, your appointment sequence and financial options will be reviewed. By the end of your first visit, our goal is that you will become knowledgeable on periodontal disease and implants, as they apply to your situation, and that all your questions are answered.
What to bring to your appointment
- Please bring your completed paperwork we have sent to you. Forms can also be conveniently downloaded below.
- The referral slip and x-rays (if you have been provided one by your general dentist)
- Your Dental Insurance card- along with the subscriber’s ID number (If you have dental insurance, we will submit your first visit and ask that they reimburse you.)
Please Note: if you Pre-Medicate for visits at your dentist, you must take your pre-med prior to your appointment here.
Due to the nature of periodontal services, it is not possible for the doctor or hygienist to see more than one patient at any given time. Since we appreciate the value of your time, we do not schedule two patients to be seen at the same time by the same professional. If you are late, it is possible that your appointment will require rescheduling. If you need to change an appointment, we request a two working day notice.
